per-database/host query tabs

not sure how easy this is to add, but making the query tabs unique per selected host+database would be useful.

sometimes i have maybe 7 or 8 tabs open which have queries in them relating to different databases on different hosts and it's hard to discern them easily (and maybe not run the wrong query accidentally against a wrong host/db)
